Staff and Supervisors, please be advised the (1) Employee Self Evaluation, (2) Supervisor Summary Evaluation and (3) Summary Evaluation & Recommendation for 2013, are now available.
From the Member's Portal, please click on "Members" and then select "UCSF Members." All three documents are located at the bottom of the page, as well as the "Rating Definitions and Percentages" document for your reference.
If you have any further questions, or would like to turn in your completed forms, please send them via email to HRsupport@immunetolerance.org.
Forms are due by October 4, 2013.
