Due to staff's imperative deadlines for "Newell" & "Nash" figures, as well as preparing updates for NSC, HR has postponed this year's Performance Evaluations. The NEW Due Date will be Monday, November 5, 2012. If you have already submitted your evaluations, you do not need to redo them. Thank you, and as always, please feel free to email any questions to HR Support.

Staff and Supervisors, please be advised the (1) Employee Self Evaluation, (2) Supervisor Summary Evaluation, and (3) Summary Evaluation & Recommendation for 2012, are now available. You may transcribe into Word, or use these PDFs as cover sheets to your comments.
From the Member's Portal, please click on "Members" and then select "UCSF Members." All three PDF forms are located at the bottom of the page, as well as the "Rating Definitions and Percentages" document for your reference.
